There you’ll find an anonymous, confidential and enjoyable survey (available in both English and Spanish) that asks questions about your physical health and overall well-being.
Based on your answers, the Web site will provide you with:

How’s Your Health, Upstate New York? is being lead by the Rochester Health Commission in cooperation with 45 area organizations, including:
- Local doctors’ organizations
- Health care systems
- Consumer organizations
- County health departments and businesses
