Discharge
Your attending physicians will work together with the nursing staff and various health specialists to plan for your discharge from the hospital and a continuation of appropriate medical care. Various members of this team will meet with you during your hospital stay and review information about your post-hospital care in detail.
Before you leave, make sure you understand your treatment plan, medications and/or supplies. Here is a checklist of reminders that may help as you anticipate your discharge.
If your physician orders a prescription for you, you can have it filled at the hospital Outpatient Pharmacy.
On the day of discharge, plan to leave the hospital by 10:00 a.m. (although sometimes your discharge may be later.) This helps us prepare for other patients who will be admitted later that day. Instructions for the person who will pick you up.
If you don' t believe you are medically ready to leave the hospital, you have the right to appeal. Ask your nurse for information on how to appeal the decision to discharge.
After you' re discharged, you may be mailed a patient satisfaction survey to fill out. We would appreciate you taking the time to let us know your thoughts as we constantly work to improve our care and services. You can, of course, write to either the Hospital Director or the Patient Relations Coordinator.
