Home Care Services
You may require the assistance of a nurse or nurse’s aide once you arrive home. A member of your health care team will assist you in selecting a home care agency to provide this service. We ask that you consider choosing Visiting Nurse Service (VNS), a Strong Health affiliate. VNS provides quality home care for everyone from newborns to seniors. You may require additional services after returning home, including rehabilitation services in a nursing home setting, physical medicine and rehabilitation, and/or home care services. Should this be the case, the social worker assigned to your unit will work with you and your family or friends to plan for referrals to those facilities best suited to your post-hospital care needs.
Many services are covered by insurance, including Medicaid, Medicare, and private insurance. Before selecting a home care service, check with your insurance carrier because they may cover only particular services or work with certain agencies.
