Getting Ready

What to Bring to Your Appointment...

Adult Medical History and Physical Form. Print this form out now, fill it out and bring it with you to your appointment (forms for children are completed during the pre-admission visit).

List of Any Required Tests. If your doctor gives you a form listing the tests you need to have, please bring it with you.

Insurance cards. You will be asked to provide your insurance information during the pre-admission visit. Also, please be prepared to make a co-payment at the time of your visit. Learn more about insurance and financial planning

List of Medications. It is very important that you inform us of any medications that you are taking and their dosages. You may either include this information on the medical history & physical form or bring a separate list to your appointment. Be sure to include the name of the medication and the dosages that you take. Do not bring your medications.

Health Care Proxy, Living Will, or Power of Attorney. If you have any of the above documents, please bring a copy so we can add this to your medical record.

 

You and your doctor have decided that you need surgery. You are ready to begin your care experience with us. Your pre-admission testing appointment (PRAT) and surgery date and time will be scheduled by your surgeon’s office. The office will contact you with appointment dates and times. If you are not contacted by the surgeon’s office, please call the office to confirm your appointments.

Your PRAT appointment may be scheduled at a variety of locations: your physician’s office, Clinton Crossing or Lac deVille medical office buildings, or at the Pre-Admission Evaluation Center (PEC) at Strong Memorial Hospital (SMH).

Why is A Pre-Admission Appointment Important?

The PRAT appointment helps us prepare you for your surgery or procedure. Your pre-admission visit should be completed at least 30 days before your surgery/procedure and at least 2 days in advance of it. This appointment allows us to:
  • Review your medical history
  • Check your vital signs (e.g., blood pressure, pulse)
  • Complete necessary laboratory tests ordered by your physician
  • Provide you with pre-procedure instructions for your surgery/procedure
Should your appointment be scheduled at Strong Memorial Hospital’s Pre-Admission Center, you can reach it by entering the Hospital’s main lobby and asking for directions at the Information Desk.

Arriving at Strong Memorial Hospital

Strong Memorial Hospital is located south of the city of Rochester, just three miles from the Rochester International Airport. The Hospital can be reached by city bus routes, three of which permit patients to arrive at one of the Hospital’s main entrances on Crittenden Blvd.
