Pre- and Post-operative Services
Our team of orthopaedic nurses, community health nurses, and social workers work with patients and their families to provide quality in-hospital care, as well as an individualized plan for care and rehabilitation after patients are discharged. This includes:
- Access to home heath care, home safety assessments,
and rehab assistance through Strong
Health's Visiting Nurse Service.
- Rehabilitation care through the Living Center at the Highlands at Pittsford, the Highlands at Brighton, and many
other partner nursing facilities in the Rochester area.
- Complete case management from social workers who have the specialized training and experience to address the unique needs of older patients.
