Social Worker

Social work in rehabilitation is directed toward maximizing patient and family use of rehabilitation care. Services help to ensure safe and reasonable discharge plans.

Our social workers will:

• Complete psychosocial evaluations and develop individualize interventions
• Summarize relevant social information for members of the health care team to
  assist in the development of treatment planning
• Provide counseling about adjustment to disability
• Evaluate and refer for substance abuse and family counseling
• Consult with and refer to vocational services, financial services and community
  resources, including a referral to a disabled peer
• Coordinate continuing care arrangements in collaboration with Community
  Health Nursing to promote ongoing recovery and adjustment
