Appointments

We do our best to make sure that you see your doctor when it's convenient for you. That's why we offer same day appointments for patients with urgent problems.

Your cooperation in arriving on time for your appointment helps us stay on schedule. If you need to cancel your appointment, please call us. When you notify us, we can offer your appointment time to our other patients. You will be charged an additional $5.00 if copay is not received at time of visit.

First Time Patients

First time patients should arrive 15 minutes early for their scheduled appointment to allow for registration. To save time, fill out our new patient history form and bring it with you.

Before your first appointment, call your doctor's office and have your records forwarded to our office. Also, remember to contact your insurance company to change your primary care provider.
