Billing and Insurance Information

Payment is due at the time of your visit. We accept cash, personal checks, or credit cards. We also accept most types of insurance, including Blue Choice, Preferred Care, Blue Choice Option, Preferred Care Option and Medicaid. If your insurance covers office visits, we will complete the necessary forms and bill the insurance company for you.

The Culver Medical Group and Strong Health are committed to providing health care to everyone. Since we are affiliated with Highland Hospital, we have access to the Hill-Burton assistance program, which covers office visit costs. Our social worker will work with the uninsured to discuss health insurance and payment options.
