Prescription Refills

If you need a refill on your medications before your next appointment, please give us at least one week advance notice. Call 585- 279-4800 between 10:00 a.m. and 11:30 a.m. and ask for your team secretary. We will need the:

1. Name of the medication
2. Dosage (mg.)
3. Pharmacy name
4. Pharmacy phone number
5. Pharmacy address
6. Prescription number
7. Directions for use

The secretary will take your message, get your chart, and give it to your provider. He/she will review the medication request within one week, and will either have the prescription ready for you to pick up at the center or will call the pharmacy with the renewal.
