Cholesterol Screening Tools For Physicians
High blood cholesterol was chosen as an initial focus of this wellness and prevention initiative because it is a major risk factor for developing heart disease, the number one killer of men and women in the United States. To address this important issue, it is recommended that cholesterol levels be checked at least once every 5 years, beginning at age 35 for men and 45 for women.
The cholesterol screening tools in this section were developed with the knowledge that the providers in our network have varied styles and approaches to patient care and, therefore, will need different tools for communicating and tracking this effort. It is important to note that the tools were developed to work in coordination with each other as a complete communication program. Regardless of which tools are appropriate for your practice, message repetition is an important factor to keep in mind.
