Initial Patient Letter - Fasting This letter is designed to encourage patients to learn more about their own cholesterol levels and recommends lipoprotein profile blood test. Fasting instructions are provided for the blood test. The letter can be used as a mailing piece or patient handout. The patient education tool - High Blood Cholesterol - What You Need to Know and a Laboratory Slip should accompany the letter. |

Results Letter This multi-purpose communication vehicle provides patients with a summary of their blood cholesterol test results, specifies any other possible risk factors for developing heart disease, and recommends next steps or follow up. |
