Practice Tools For Cholesterol Screening

 

Cholesterol Screening Chart Review Tool

This piece was designed to prompt office staff (nurses, front desk staff and physicians) to review the patient medical record for the date of the patient's last cholesterol screen.

This tool will assist in identifying patients needing a cholesterol screen and will act as a prompt for the physician/provider to recommend a follow-up action for the patient.

Cholesterol Screeening Stickers

This tool was designed for office staff (nurses, front desk staff and physicians) to place on the outside of the medical record. The date of the patient's most recent cholesterol screen will be easily identifiable. If the prompt is blank it is a reminder for the staff to follow-up with the patient.
