Appointments

The providers in our office see patients by appointment only. If you have an emergency and feel that urgent care is indicated, please call our office for immediate instructions, dial 911, or go to Strong Memorial Hospital's Emergency Department for care.

Each physician reserves open appointment slots for patients needing urgent care daily. However, most non-urgent visits can be scheduled within one to two weeks of your request, depending on which doctor you see.

If your physician would like for you to see a specialist outside of our office for a consultation or a test of any kind, the secretary will give you the complete name, address, and telephone number of the facility that you have been referred to so you can make your own appointment that fits into your schedule. In urgent, isolated cases the secretary will make outside office appointments for patients.

Forms

If your physician needs to fill out a form for you, please make certain that the entire patient portion has been filled out and that a return envelope or complete instructions are available as to where our office needs to send or fax the form. Please allow our office at least 48 hours to complete any form.
