Making An Appointment

For New Patients

Your first step is to call the office and express your interest in becoming a patient. We will ask that you complete a “new patient” information packet, which asks you to provide information about your medical history, medications, etc. If we can accept you into our practice, we will ask you to contact your current primary care provider and ask that he or she forward your records to us before your first appointment. We will also ask you to inform your insurance company, if we become your new primary care provider. First-time patients should bring all insurance cards and arrive 15 minutes early to allow for registration.

For Current Patients

We do our best to make sure that you see your doctor when it's convenient for you. We also try to keep our appointment times flexible to meet your needs for sick visits. We will make every effort to schedule a sick appointment on the same day that you call. Your cooperation in arriving on time for your appointment helps us stay on schedule. If you cannot keep an appointment, please give us one day's notice, so that we can accommodate the urgent needs of another patient.

Despite our best efforts, emergencies can cause delays. If your appointment time is adversely affected by an office or hospital emergency, we will try to notify you.
