What a Calcium Score Measures
A coronary artery calcium (CAC) score measures the amount of calcified plaque in the arteries that supply your heart, using a low-dose CT scan. The result is reported in Agatston units, from 0 (no detectable calcium) into the thousands [1].
Calcium only shows up in coronary arteries where atherosclerosis has already started. That makes the score a direct marker of disease, not an estimate. Cholesterol panels and risk calculators predict what might happen; a calcium score shows what has already happened in your arteries. In the MESA study, which followed more than 6,800 adults, calcium score outperformed every traditional risk factor at predicting coronary events [6].
Age is the strongest driver of calcium scores in the general population. A score of 200 means something very different at 45 than it does at 75, which is why age- and sex-specific reference values matter when you read your result.
Average Calcium Score by Age and Sex
The average (median) calcium score is 0 for men under about 50 and for women under about 60. After that, typical scores climb steadily with age: the median for men reaches roughly 25 by the early 60s and over 100 by the early 70s, while women trail men by roughly a decade.

One caution before you compare yourself to these numbers: "average" is not the same as "healthy." Atherosclerosis is so common in older adults that the average 70-year-old man carries a meaningful plaque burden. The ideal score at every age is 0.
Average Calcium Score at 40
The average calcium score for a 40-year-old is 0, for both men and women. Most people in their 40s have no detectable coronary calcium, so any score above 0 at this age puts you above the norm for your peers.
That's what makes a positive score in your 40s so significant. A 42-year-old man with a score of 40 might be in the 90th percentile for his age, meaning his arteries look decades older than his birth certificate. MESA follow-up data show that people with any calcium at a young age face substantially higher lifetime risk than peers who stay at 0, even when their absolute score is modest [3].
Screening in your 40s makes the most sense if you have a family history of early heart disease, borderline-high cholesterol, or you're deciding whether to start a statin. If your score is 0, you've bought yourself time. If it isn't, you've caught the disease at the stage where treatment changes outcomes most.
Average Calcium Score at 50
The average calcium score for a 50-year-old man is roughly 0 to 5; for a 50-year-old woman it is still 0. The 50s are the decade when scores begin separating: about half of men aged 50–59 have at least some detectable coronary calcium, while roughly three quarters of women remain at 0.
For a man of 50, a score under about 25 is near the middle of the pack, a score over about 65 lands in the top quarter, and a score in the hundreds is well above the 90th percentile. For a woman of 50, any positive score is above average, and a score over about 20 is already in the top 10% for her age.
This is also the decade where the test earns its keep as a statin tie-breaker. The 2018 AHA/ACC cholesterol guideline explicitly recommends calcium scoring for adults at intermediate risk when the statin decision is uncertain: a score of 0 supports holding off, and a score of 100 or higher (or ≥75th percentile) favors starting [4].
Average Calcium Score at 60
The average calcium score for a 60-year-old man is roughly 25; for a 60-year-old woman it is still 0. By age 60, roughly two thirds of men have a positive score, while most women have not yet developed detectable coronary calcium.
Reference points for a 60-year-old man: 0 is better than average and carries an excellent near-term prognosis, ~25 is the middle of the distribution, ~130 marks the top quarter, and ~430 or higher is in the top 10%. For a 60-year-old woman, ~20 already reaches the top quarter and ~130 the top 10%.
Percentile matters as much as the raw number here. A 60-year-old man with a score of 90 has an "average-ish" absolute burden but sits below the threshold many guidelines use for automatic statin recommendation; a 60-year-old woman with the same 90 is far above the norm for her sex and age, and her physician will usually treat it more aggressively.

Average Calcium Score at 70 and Older
The average calcium score for a 70-year-old man is roughly 130, and for a 70-year-old woman roughly 10. Past 75, the male median climbs above 200. At this age a score of 0 is genuinely uncommon, and genuinely good news: older adults with zero calcium have event rates comparable to much younger people [3].
Interpreting a score at 70 is less about "do I have plaque?" (most men do) and more about how much, and what to do about it. A 70-year-old man at 100 is below average for his age; the same score in a 70-year-old woman is above the 75th percentile. Scores over 400 at any age indicate extensive disease and usually prompt a cardiology consult, aggressive lipid management, and sometimes further testing for silent ischemia.
Testing past 70 is most useful when the result would change management: deciding on statin intensity, weighing aspirin, or evaluating vague symptoms. If you're already on maximal preventive therapy after a heart attack or stent, a screening calcium score adds little.
Why Percentiles Beat Averages
A calcium score percentile compares your score against people of the same age, sex, and ethnicity, and it often tells you more than the raw number. The raw score measures absolute plaque burden, which tracks your near-term event risk. The percentile tells you how fast your arteries are aging relative to your peers, which tracks your long-term trajectory.
A 45-year-old man with a score of 30 has a low absolute burden but sits around the 90th percentile: his near-term risk is modest, but his arteries are calcifying early, and prevention now pays off for decades. A 78-year-old man with a score of 150 has a higher absolute burden but sits below the median for his age. His arteries are aging normally.

If Your Score Is Above Average for Your Age
Treatment for an above-average calcium score means statin therapy, blood pressure control, and lifestyle change rather than surgery. Calcified plaque cannot be removed, and the score itself won't go down, but treatment stabilizes plaque and cuts the risk that it ruptures and causes a heart attack.
The regimen is well established: a statin (with intensity matched to your score and risk factors), LDL targets confirmed by follow-up labs, blood pressure below 130/80, smoking cessation if it applies, and regular aerobic exercise.
