Bad habits or lack of good habits in the areas of nutrition, weight loss, fitness and smoking cessation are a contributing factor to over half of all deaths in our community. If you're willing to make a behavior change in your life, we've developed tools to help you reach your goal.

The Referral and Resource Guide is divided into four sections:
