Information for New Patients

To prepare for your consultation with a spine doctor, please plan on bringing the following items to your visit:

  • A list of your current medications
  • Information on what treatments and medications have been tried in the past
  • Films from outside hospitals–We will obtain films from University Medical Imaging (UMI) and Strong Memorial Hospital for you prior to your visit.
  • Medical records from prior visits that you think would be helpful
  • Insurance information and cards 

Save Time  – Complete our Spine Patient Questionnaire beforehand and bring your completed form with you to your visit.

What should you expect?

As a patient you will receive a full evaluation from one of the Spine Center’s physiatrists, a specialized non-operative physician, who determines the exact location of the problem, the nature of the injury and the course of action for further diagnosis and treatment. For the duration of your stay, a physician will coordinate all aspects of care. Patients with urgent conditions can usually be seen that day. If surgery is indicated, the visit will include a surgical consultation. 

Every patient receives a treatment plan created specifically for him/her and is supervised by a physiatrist. This treatment goes beyond basic pain management by employing therapeutic methods that treat the specific source of the problem, providing a specific treatment program for reduction in pain and improvements for a more active lifestyle.
