Making the Transition
If you or your loved one is leaving Strong Memorial Hospital, Highland Hospital, or Golisano’s Children Hospital and need help understanding your home health care options, we can help. Our triage nurses at all hospitals will assist you in determining the right home care for you or your loved one.
Ask a hospital staff nurse to contact the VNS onsite nurse before you leave the hospital.
